Excellentiam operates as an elder-care and healthcare aggregator, facilitator and care-coordination organisation. Depending upon the services requested, we may coordinate or facilitate services through independent third-party Service Providers, including nurses, attendants, caregivers, physiotherapists, palliative-care professionals, hospitals, diagnostic centres, pharmacies, medical-equipment providers and other healthcare professionals or organisations.
Depending on the nature of the service, we may collect identification and contact information; Elder/Patient information such as medical history, conditions, allergies, medicines, prescriptions, reports and care requirements; service information such as services requested, Service Provider assigned, visits, attendance, invoices, payments and complaints; and Website/technical information such as IP address, browser/device information, pages visited and technical logs.
We may process personal information for responding to enquiries; registering Elders/Patients; assessing service requirements; coordinating care; identifying Service Providers; communicating with Elders, Patients and family members; coordinating nurses, attendants and caregivers; coordinating hospital assistance, physiotherapy, palliative care and allied services; coordinating medical equipment; communicating doctors’ prescriptions or instructions to relevant Service Providers where authorised; arranging appointments and visits; maintaining records; preparing invoices; handling complaints; preventing fraud; maintaining security; complying with law; protecting safety; improving services and Website; and communicating permitted service information.
Because Excellentiam provides and coordinates elderly-care and healthcare-support services, we may receive information concerning an Elder’s or Patient’s medical condition, prescriptions, reports, treatment and care requirements. Such information shall be treated as confidential and used primarily for providing or coordinating the requested service, maintaining service records, communicating with authorised persons and complying with applicable law. Excellentiam does not independently diagnose or prescribe treatment.
We may collect information about children, spouse, relatives, Next of Kin, Legal Heirs, authorised representatives and emergency contacts where reasonably required for service, communication, emergency, administrative or legal purposes.
For nurses, attendants, caregivers, physiotherapists, healthcare professionals, agencies and other Service Providers, Excellentiam may collect name, contact information, address, identification, qualifications, registration/licence details, experience, references, background-verification information, service history, attendance and payment information, as reasonably necessary.
Information may be collected directly from you through Website forms, telephone, email, WhatsApp or other communication channels, physical forms, service agreements, enrolment forms, family members or authorised representatives, doctors or hospitals where authorised or necessary, Service Providers, or other lawful means.
Where consent is required under applicable law, Excellentiam shall seek consent in an appropriate manner. Consent should be free, specific, informed and unambiguous and relate to the stated purpose. Where applicable, consent may be withdrawn through the mechanism provided by Excellentiam.
Withdrawal does not affect processing already lawfully undertaken and may affect our ability to provide services where the information is necessary.
We may share relevant personal information with nurses, attendants, caregivers, physiotherapists, doctors, hospitals, diagnostic centres, pharmacies, medical-equipment providers, other Service Providers, authorised Excellentiam personnel, payment and technology providers, professional advisers, insurers/administrators where relevant, and government or law-enforcement authorities where required or permitted by law. Disclosure shall be limited to information reasonably necessary for the relevant purpose.
Because Excellentiam operates as an aggregator and care coordinator, sharing relevant information with a Service Provider may be necessary to arrange and deliver the requested service. Service Providers may be required to comply with confidentiality and data-handling obligations under their agreements with Excellentiam.
Excellentiam may disclose personal information where reasonably necessary to comply with law, respond to lawful government requests, comply with court orders, cooperate with law-enforcement investigations, prevent fraud, investigate suspected theft, abuse or misconduct, protect safety, or protect Excellentiam’s legal rights.
In an emergency involving an Elder or Patient, Excellentiam may disclose relevant information to family members, Next of Kin, emergency contacts, doctors, hospitals, ambulance/emergency services or other appropriate persons where reasonably necessary to protect life, health or safety, subject to applicable law.
Excellentiam shall implement reasonable technical and organisational safeguards appropriate to the nature of personal information processed. Safeguards may include access controls, password protection, restricted access, secure storage, authentication, confidentiality obligations, backup procedures, monitoring and logging. No electronic system or transmission can be guaranteed completely secure.
Where a personal-data breach occurs, Excellentiam shall take reasonable steps to contain, assess, investigate and address the incident and provide notifications or take other actions where required under applicable law.
Excellentiam shall retain personal information only for as long as reasonably necessary for providing or administering services, legal/regulatory compliance, accounting and taxation, resolving disputes, preventing fraud, establishing or defending legal claims, or other lawful purposes. When no longer required, information may be deleted, anonymised or securely disposed of, subject to legal requirements.
Our services are primarily intended for adults and elderly persons. We do not knowingly seek personal information from children except where lawfully provided by a parent, guardian or authorised person and reasonably necessary for a service. Where applicable law imposes additional requirements, Excellentiam shall comply.

Where an Elder or Patient is unable to independently communicate, consent or exercise applicable rights, Excellentiam may communicate with the person’s legally authorised representative, Next of Kin or other authorised person, subject to applicable law.
Excellentiam shall take reasonable steps to maintain confidentiality of personal and healthcare information. Employees, representatives and Service Providers may be subject to confidentiality obligations appropriate to their role. Confidential information may be disclosed where necessary for service delivery, emergency response, legal compliance, protection of safety or other lawful purposes.
Privacy-related complaints or concerns may be submitted to the Privacy/Grievance Contact identified below. Excellentiam shall review and respond to privacy-related grievances in accordance with applicable law and internal procedures.
Requests concerning access, correction, erasure, withdrawal of consent or other applicable data rights should contain sufficient information to identify the individual and process the request.
